CAN I SEE A PHYSICAL THERAPIST WITHOUT A SCRIPT OR REFERRAL?
In Colorado you can see a physical therapist without a referral. Medicare requires a doctor’s script to continue treatment after the first visit.
WHAT SHOULD I WEAR?
Wear comfortable clothing and footwear that allow you to move easily, and allows the physical therapist to view the area we’re working on. For example: T-shirt or tank top and shorts. We do provide shorts and gowns if necessary for your treatment.
WHAT SHOULD I EXPECT ON MY FIRST APPOINTMENT AND FOLLOW UPS?
First Appointment: Your initial evaluation will be 40 minutes. Please bring information that may be helpful to your therapist, such as your prescription from your doctor, protocols/directions from surgeon if you are recovering from surgery.
Your therapist will review your medical history and symptoms, conduct a thorough examination to determine what factors may be contributing to your pain, discuss your goals and expectations for physical therapy, and develop a plan of care.
We ask that you be on time for this appointment so that you get the full benefit of your first treatment. If you prefer to fill paperwork out upon arrival, please plan to arrive 15 minutes early. We recommend parents accompany patients under the age of 18 for the first treatment.
Follow Up Visits: Appointments will be 40-45 minutes in length, and include assessment of your progress, followed by treatment. Treatment may include manual interventions, as well as appropriate modifications to your exercise program.

FINANCIAL POLICY AND INSURANCE
Our policy is to contact your insurance company to verify your benefits. We suggest you also contact them so you are aware of their requirements regarding authorizations, referrals, policy exclusions, and your financial responsibility. Copayments and Coinsurance are due at time of service. If your deductible has not been met, there will be a minimum payment of $70. Some costs may be incurred that are not covered by your insurance, such as purchasing exercise equipment or electing to try dry needling as part of your treatment.
CANCELLATION POLICY
We send email reminders the day before your appointment (please notify us if you wish to not receive these). Out of respect to our therapists and other patients who may be on a waiting list, please give 24-hours’ notice to cancel appointments. There is a $35 charge for first time no shows or late cancelled appointments.
